Objectif. Évaluer l’intérêt de l’échographie thyroïdienne dans l’hypothyroïdie.
Méthodes. Nous avons étudié 421 cas d’hypothyroïdie franche et 1 845 cas d’hypothyroïdie infraclinique chez l’adulte. Tous les patients ont eu une recherche des anticorps antithyroïdiens et une exploration échographique du corps thyroïde.
Résultats. L’échographie a été déterminante pour l’exploration des nodules non palpables. Neuf patients ayant une hypothyroïdie infra-clinique avaient un cancer dont 4 un cancer non palpable. L’aspect échographique de ces neuf nodules était suspect dans six cas. Dans l’hypothyroïdie franche, les deux nodules cancéreux opérés étaient palpables et suspects par leur aspect échographique.
L’échographie a permis l’étude de la biométrie thyroïdienne. En particulier, l’atrophie a été observée dans près d’un tiers des cas dans les deux types d’hypothyroïdie.
L’échographie a permis d’améliorer le diagnostic de la thyroïdite auto-immune de 92 à 98 % des cas dans l’hypothyroïdie franche, et de 70 à 79 % des cas dans l’hypothyroïdie infraclinique.
Conclusion. L’intérêt essentiel de la pratique systématique de l’échographie thyroïdienne a été de dépister des cancers non palpables au cours de l’hypothyroïdie infraclinique. Le second intérêt a été d’améliorer le diagnostic étiologique de l’hypothyroïdie, plus souvent dans l’hypothyroïdie infraclinique, et de surveiller l’évolution d’un goitre ou de nodules initialement non suspects.
Disparity of ultrasonography relevance in overt versus subclinical hypothyroidism
Purpose. The assessment of thyroid ultrasonography in hypothyroidism.
Methods. We studied 421 cases of overt hypothyroidism and 1845 cases of subclinical hypothyroidism. All patients were screened for thyroid antibodies and underwent an ultrasonographic thyroid examination.
Results. Ultrasonography was useful for the assessment of non palpable nodules. Nine patients with subclinical hypothyroidism had cancer: 4/9 patients had a non palpable cancer. Ultrasonography first displayed suspicious aspects in six patients. Two patients with overt hypothyroidism showed a palpable cancer with initially ultrasonographic suspicious aspects.
Ultrasonography facilitated the study of thyroid biometry. Particularly, atrophy was encountered in almost one third of cases in the two types of hypothyroidism.
Ultrasonography led to improved diagnosis of autoimmune thyroiditis from 92 to 98 percent of cases in overt hypothyroidism and from 70 to 79 percent of cases in subclinical hypothyroidism.
Conclusion. The primary relevance of thyroid ultrasonography was the screening of some non palpable cancers in subclinical hypothyroidism. The secondary relevance was the improvement in the etiologic diagnosis of hypothyroidism, more often in subclinical hypothyroidism, and the follow-up of thyroid enlargement and initially non suspicious nodules.
