L’évaluation des résultats d’une technique chirurgicale exige que le risque de complications soit correctement étudié. L’objectif de ce travail était de savoir si la coeliochirurgie gynécologique, lorsqu’elle est réalisée pour une pathologie bénigne (ligature de trompes, grossesse extra-utérine, kyste ovarien, hystérectomie, myomectomie …), exposait ou non à un risque de complications plus important que l’intervention par laparotomie. Pour cela nous avons effectué une méta-analyse spécifiquement centrée sur le problème des complications en prenant en compte tous les essais prospectifs randomisés comparant la coeliochirurgie et la laparotomie pour une pathologie gynécologique bénigne. Les résultats de cette métaanalyse montrent que le risque global de complications était significativement moins important pour les patientes opérées par voie coeliochirurgicale (RR 0.59 ; 95% CI 0.50 - 0.70). Par ailleurs il n’existait pas de différence statistiquement significative quant au risque de complications majeures en fonction de la voie d’abord (RR 1.0 ; 95% CI 0.60 – 1.65). Le risque de complications mineures était significativement moins important chez les patientes opérées par voie coelioscopique (RR 0.55 ; 95% CI 0.45 – 0.66).
Is gynaecological laparoscopic surgery dangerous ?
An appropriate study of possible complications is necessary to evaluate the results of a surgical technique. The aim of this study was to find out if gynaecologic laparoscopy, when performed for benign gynaecologic pathology (tying of the Fallopian tubes, extra uterine pregnancy, ovarian cyst, hysterectomy, myomectomy) would expose patients to an increased risk of complications compared to laparotomy. A meta-analysis specifically focused on complications, including all randomized prospective trials comparing laparoscopy and laparotomy for benign gynaecological pathology was conducted. Results of this meta-analysis show that global risk of complications was significantly less important for patients operated on through laparoscopy than for patients operated on through laparotomy (RR 0.59 ; 95 % CI 0.50-0.70). Moreover, there was no statistically significant difference concerning risk of major complications in relation to surgical procedure (RR1.0 ; 95% CI 0.60- 1.65). The risk of minor complications was significantly less important in patients operated on through laparoscopy (RR 0.55; 95% CI 0.45-0.66).

Les dissections isolées et spontanées de l'artère rénale sans dissection aortique associée sont une cause inhabituelle d'hypertension artérielle. L'auteur présente une série comportant 23 patients opérés. Ces dissections peuvent compliquer une fibrodysplasie artérielle mais, dans la moitié des cas environ, la cause est mal connue. Elles entraînent une hypertension artérielle grave, d'apparition brusque, qui résiste parfois au traitement médical. Le diagnostic repose essentiellement sur l'angiographie. Le traitement médical isolé est indiqué lorsque l'hypertension artérielle est modérée et facilement contrôlable médicalement. Le traitement chirurgical est indiqué en cas d'hypertension résistante au traitement. La néphrectomie est réservée aux reins gravement altérés et aux dissections s'étendant aux branches intra-rénales. La reconstruction artérielle est le traitement idéal ; elle a grandement bénéficié des techniques de chirurgie extra-corporelle. Les résultats du traitement chirurgical sur la tension artérielle sont très favorables à condition que l'on ait pu rétablir une vascularisation normale du(des) rein(s) et que le parenchyme rénal soit sain. Le suivi des patients doit être prolongé en raison de la possibilité d'une dissection controlatérale.
Isolated spontaneous dissection of the renal artery (ISDRA) is an
uncommon cause of arterial hypertension. In our experience, it accounts
for 1.5 % of our surgical reconstructions of the renal artery
Purpose : The aim of this study was to present a homogeneous series of isolated spontaneous dissections of the renal artery operated on by the same surgeon, to assess the feasibility of renal artery reconstruction and to study the results of surgical treatment. Extension of an aortic dissection to the renal arteries as well as dissections complicating transluminal angioplasty were excluded. Patients and Methods : During a 21-year period (1978-1998), 22 patients (17 men, 5 women) with spontaneous dissection of the renal artery were operated on. Their mean age was 41 ± 8.4 years. All patients presented with uncontrollable arterial hypertension of recent and abrupt onset. In ¾ of them, the initial symptom was lumbar or flank pain. These patients underwent the usual investigations performed in subjects with hypertension. Arterial opacification was carried out with standard abdominal aortography or intra-arterial digital angiography in the majority of the cases. Recent imaging techniques (scan 3 D, angio MRI) will probably replace the former investigations in the future (figure 1). Diagnosis was sometimes difficult when radiological images were atypical. Due to bilateral lesions (3 patients) or combined operation (1 patient), surgical treatment consisted of 8 nephrectomies (6 total, 2 partial) and 17 arterial repairs (in 16 patients) carried out by conventional in situ surgery (6 cases) or by extracorporeal repair (11 cases). The high rate of the latter technique was due to the severity of the lesions with frequent extension to several terminal branches of the artery (ies). Results : There was no postoperative mortality or morbidity in this series. Arterial hypertension was cured in 9 patients (41 %), improved in 11 (50 %) and unchanged in 2 (9 %). The anatomical results of repairs were excellent in 13 patients (81 %) (figures 2, 3 and 4) and incomplete in 3 (19 %), because of renal remaining damages (segmental atrophy, notching, irregularities of renal parenchyma). During long-term follow-up, 1 late thrombosis of a repaired polar artery (at 2 years) and 1 spontaneous dissection of the contralateral renal artery (at 5 years) occurred; the latter patient was successfully treated with anti-hypertensive drugs. In 8 patients, late angiographies showed that the results of reconstructions remained stable with time. Two patients had impaired renal function before the operation. Serum creatinine level was slightly elevated in one patient (170 μg/L) and rose to 400 μg/L in the other. The first patient regained normal renal function after repair of the renal artery. In the other, renal function remained slightly impaired (serum creatinine of 140 μg/L) but stable for 10 years. Conclusions : Surgical treatment of isolated spontaneous dissection of the renal artery is indicated in patients presenting with severe uncontrollable hypertension in spite of heavy medical treatment. Arterial repair is the ideal treatment. It is often possible even when complex lesions are present. In such cases ex vivo surgery affords great safety and ease of repair. Results concerning control of hypertension are satisfactory. Anatomical results remain stable with time.

Les conflits ulno-lunaires secondaires à des séquelles de fractures du radius sont dus à l'inversion de l'index radio-ulnaire distal par raccourcissement relatif du radius. Cette variance ulnaire positive entraîne à terme un conflit entre la tête de l'ulna et la face articulaire proximale du semi-lunaire avec altération des surfaces cartilagineuses carpiennes. Les choix thérapeutiques reposent sur l'évaluation spécifique de cette articulation dans une analyse clinique et para clinique et l'apport récent de l'arthroscopie du poignet permet souvent le diagnostic et le traitement de façon peu invasive.
Ulno carpal abutments : the state of the art and the place of arthroscopy
Ulno carpal abutments secondary to the sequels of a fracture of the radius are often due to the inversion of the distal radio ulnar index by relative shortening of the radius. This positive ulnar variance eventually leads to an abutment between the head of the ulnar and the proximal articular face of the lunate with alteration of the cartilaginous carpal surfaces. The choice of therapy is based on a specific evaluation of this joint in a clinical and para clinical analysis and the recent contribution of wrist arthroscopy makes diagnosis and treatment possible in a less invasive way.

Entre 1991 et 2000, 39 patients ont été traités chirurgicalement pour un névrome de Morton. La série comprenait 33 pieds chez 30 patients. La population féminine était prédominante avec une moyenne d’âge de 48 ans. La neurectomie a été réalisée chez tous les patients. Avec un recul dépassant 3 ans, 30 pieds soit 36 névromes ont été revus. Trente quatre ont été améliorés sur le plan de la douleur. Seuls 10 patients avaient une modification du chaussage. Quatorze hypoesthésies commissurales et 7 hypoesthésies commissurales et pulpaires ont été relevées. La cicatrice était indolore dans 31 cas, rétractile dans 3 cas. La neurectomie a permis d’obtenir un bon résultat dans 90% des cas.
Treatment of Morton's Neuroma. A review of 36
cases treated by neurectomy.
Between 1991 and 2000, 39 patients having Morton’s neuroma were treated surgically.The series included 33 feet in 30 patients. Female predominance was clear with a mean age of 48 years. Neurectomy was performed in all cases. With a follow-up period of over 3 years, 30 feet were examined, which represented 36 neuromas. Thirty four were painless. Shoe wear was normal in the whole series except for 10 patients. Fourteen commissural hypoesthesia and 7 commissural and pulpar hypoesthesia were noted. Scars were painless in 31 cases, hyperkeratosic in 3 cases. Neurectomy led to a total disappearance of pain in 90% of the cases.

En l’absence de transplantation hépatique d’urgence la mort survient dans 80 % des cas d’hépatite fulminante (150 en France chaque année). Pendant l’attente de la transplantation une assistance hépatique artificielle est nécessaire. Parmi les nombreuses méthodes utilisées, les systèmes de foie bioartificiel sont un progrès manifeste. Les résultats d’un essai clinique de phase I/II (10 malades inclus) sont rapportés. Tous les patients ont pu être transplantés secondairement et 8 étaient vivants avec un recul supérieur à 2 ans.
Bioartificial liver
Without emergency liver transplantation death occurs in 80% of the cases of fulminant hepatitis (150 per year in France).The use of artificial liver assist devices is necessary while awaiting transplantation. Among the various methods used, bioartificial liver systems are an obvious progress. Results of a clinical phase I/II trial (including 10 patients) are analysed. All patients subsequently underwent liver transplantation and were alive with a 2 yearfollow- up.

La transplantation hépatique par donneur vivant est une alternative pour pallier la pénurie d’organes. Elle a été réalisée 675 fois en Europe entre octobre 1992 et juin 2002 (399 fois chez des enfants et 276 fois chez des adultes). Ses résultats sont comparables à ceux de la transplantation conventionnelle. Son développement laisse espérer une réduction de la mortalité des patients en attente de transplantation.
Current state of adult living donor transplantation : data
from the European Liver Transplant Register
Living donor liver transplantation is an alternative to compensate the lack of organs. It was performed 675 times in Europe between October 1992 and June 2002 (399 times in children and 276 times in adults). Results are comparable to those of conventional liver transplantation. Its development will hopefully lead to a reduction of mortality for patients awaiting transplantation.

Nous avons une bonne expérience en traumatologie de l'amputation de l'arrière pied avec arthrodèse tibio-calcanéenne, dérivée de la technique de Pirogoff. Nous en rappellerons ici les principes et en particulier l'intérêt de la verticalisation du calcaneum. Cette intervention offre de nombreux avantages ; l'accourcissement est minimal permettant la marche sans orthèse, pour le lever nocturne par exemple, une excellente qualité du moignon distal utilisant la coque talonnière "en place". La fixation de l'arthrodèse se fait en règle par un vissage direct. Lorsqu'on est obligé de réaliser cette intervention en milieu septique, la poursuite ou l'aggravation de l'infection, le démontage des ostéosynthèses, sont des complications fréquentes et graves et peuvent obliger à des amputations plus proximales. L'utilisation dans ce cas du fixateur dynamique d'Ilizarov a été pour nous d'un très grand secours. En effet, la méthode "d'Ilizarov" grâce à la reprise très précoce de l'appui, permet d'obtenir les micro mouvements bénéfiques qui stimulent la vascularisation. Cette méthode est largement utilisée pour les allongements des membres et permet même le traitement de certaines artérites périphériques. Nous avons utilisé le fixateur circulaire d'Ilizarov dans 12 cas de maux perforants plantaires gravissimes souvent multi opérés, lorsque les amputations du métatarse, voire du tarse antérieur étaient dépassées. Il s'agissait dans tous les cas de patients ayant une artérite diabétique sévère. Tous les opérés ont pu reprendre un appui très précoce, ce qui est essentiel pour la méthode. Toutes les arthrodèses ont fusionné en moyenne en deux mois. Après l'ablation du fixateur, l'orthèse définitive a été réalisée, permettant un chaussage quasi normal. Nous n'avons observé aucune récidive infectieuse, et surtout la trophicité du moignon a toujours été étonnamment bonne.
Pirogoff amputation for forefoot severe chronic deep infection
in patients with diabetis mellitus: interest of a fixation by Ilizarov
system
Surgical results and functional outcome of a derived Pirogoff amputation procedure using external fixation with Ilizarov technique were studied retrospectively in 12 patients with severe diabetes mellitus who had needed amputation for forefoot severe chronic deep infection. Follow-up was over one year for all patients. All patients had severe peripheral vascular disease. The amputation was carried out as described by Pirogoff, but the calcaneous was bound on the end of the tibia with an external Ilizarov system. The patients were allowed to walk over the stump with full weight bearing immediately. Fusion of the arthrodesis was achieved in all 12 cases with an average of 2 months (45 days to 5 months) and we did not observe any case of stump dislocation. The stump had a good trophicity in all cases and there was no recurrence of infection. After removal of the Ilizarov system, patients were able to wear shoes with adequate prostheses and to deambulate successfully. The Pirogoff amputation has the advantage of creating a strong weightbearing stump that is the same length as the patient’s normal leg. But stump dislocation is an important complication of this technique. To avoid this problem, the use of splinting of the stump in the postoperative period is mandatory but it prolongs the patients’ postoperative rehabilitation. The Ilizarov system is solid enough to allow immediate postoperative full weight bearing and improves the bone fusion by micromobility. It makes the care of an eventual wound easier.

Depuis Péan en 1893 et Judet en 1947, l’arthroplastie prothétique de l’épaule a considérablement évolué grâce aux travaux de Neer qui propose, dès 1951, un concept nouveau de prothèse à glissement dont la mise en place doit, d’une part, respecter et réparer les parties molles et, d’autre part, être suivie d’une rééducation postopératoire programmée qui fait 50 % du résultat fonctionnel. Toutes les prothèses actuelles, modulaires et adaptables reposent sur ces deux principes de base. Le choix entre prothèse humérale simple (30 %) et prothèse totale (70 %) dépend des informations fournies par l’imagerie préopératoire sur l’état des tendons de la coiffe des rotateurs et le stock osseux glénoïdien. La chirurgie de la prothèse d’épaule est une chirurgie des parties molles qui demande une technique très minutieuse, qui va induire directement la qualité du résultat fonctionnel, en particulier après mise en place pour une fracture récente et complexe de l'extrémité supérieure de l’humérus du sujet âgé. Les meilleurs résultats fonctionnels sont obtenus dans l’omarthrose primitive et l’ostéonécrose idiopathique de la tête humérale. Dans la polyarthrite rhumatoïde, les résultats sont constants sur la douleur tandis que la récupération de la mobilité dépend du stade de la maladie. Les résultats dans l’omarthrose post traumatique et post instabilité dépendent de l’état préopératoire des parties molles. L’omarthrose excentrée avec rupture massive de la coiffe est l’indication la plus controversée quant au choix du type de prothèse, à glissement ou inversée. Les complications de la prothèse d’épaule sont liées à des indications mal fondées, à la technique opératoire et à la pathologie causale. La prothèse d’épaule est aujourd'hui une alternative thérapeutique fiable à long terme avec 93 % de survie après dix ans et 87 % après 15 ans, à la condition qu’elle soit mise en place et suivie par une équipe spécialisée ayant une bonne expérience de cette chirurgie.
Shoulder arthroplasty in 2002
Since Péan in 1893 and Judet in 1947, shoulder arthroplasty subsequently evolved with the original concept of non constrained prothesis proposed by Neer in 1951. All actual modular and adaptable shoulder arthroplasties are based on this concept, which associates both repair of soft tissues and postoperative comprehensive rehabilitation program. Preoperative imaging give information on the status of rotator cuff and the glenoid bone stock and allows preoperative choice between humeral or total shoulder arthroplasty. Shoulder arthroplasty needs a good management of soft tissues and a meticulous surgical technique, particularly in the treatment of acute complex fractures in elderly patients. Best functional results are obtained in primary osteoarthritis and avascular necrosis of humeral head. In rheumatoid arthritis, pain is lessened and function is improved. Results in post traumatic and post instability osteoarthritis depend on preoperative status of soft tissues. Choice of type of prosthesis is actually very controversial in cuff tear arthroplasy. Complications of shoulder arthroplasty depend on non selective indications, operative technique and aetiology. In 2002, shoulder arthroplasty is a reliable long-term therapeutic alternative with a 90% survival after 10 years and 87 % survival after 15 years, when performed by an experienced team in the field of shoulder’s surgery and rehabilitation.
