L’insuffisance cardiaque chronique est une pathologie majeure dont
la prévalence augmente régulièrement. En dépit des nombreux
progrès réalisés dans la compréhension des mécanismes de
l’insuffisance cardiaque, le pronostic de l’insuffisance cardiaque
sévère reste sévère. La transplantation est aujourd’hui le traitement
de choix. Cependant, le faible nombre de greffons disponibles
impose des critères de sélection rigoureux des candidats. Suivant la
ligne de conduite définie par Monsieur Cachera, tout un faisceau
d’arguments objectifs est utilisé pour sélectionner les candidats
optimaux.
Cardiac transplantation : patients’ selection
Chronic cardiac failure is an increasing problem. Despite significant
progress in the understanding of the physiopathology and improved
medical management, prognosis remains severe. Cardiac
transplantation is now the gold standard in the management of the
patients who become unresponsive to optimal and tailored medical
therapy. Organ shortage is an increasing problem which makes
mandatory a strict patient selection.
The spirit of Jean-Paul Cachera, one of the pioneers in this
adventure, is still alive in the definition of the optimal protocols and
the management of the transplantation program at Henri Mondor’s
Hospital.
