Avant toute exploration spécifique de l'incontinence anale (IA), un
examen morphologique comprenant soit une coloscopie complète,
soit une rectosigmoïdoscopie associée à un lavement baryté, doit être
discuté chez tous les patients présentant une IA à rectum vide, et
cela en fonction du contexte clinique (antécédents familiaux de cancer
colique, âge, manifestations cliniques associées…). Les explorations
spécifiques de l'IA vont avoir pour but de répondre à plusieurs
questions:
1/ Existe-t-il une incompétence sphinctérienne responsable de
l'IA ? La manométrie anorectale, en déterminant la valeur de la
pression anale de base ainsi que l'amplitude et la durée de la contraction
volontaire du sphincter anal externe, permet de savoir s'il existe
une incompétence du sphincter anal interne (pression basale) et/ou
du sphincter anal externe (contraction volontaire). L'examen clinique
peut parfois être aussi performant que la manométrie anorectale pour
répondre à cette question. Néanmoins, l'examen manométrique permet
également d'évaluer la capacité fonctionnelle rectale, sa compliance,
et de savoir si un dysfonctionnement rectal n’est pas en
cause dans la physiopathologie de l'IA.
2/ Quelle est l'origine de l'incompétence sphinctérienne ? Il existe
deux origines possibles à l'incompétence sphinctérienne: une lésion
anatomique sphinctérienne et/ou une atteinte neurologique. L'échographie
endo-anale permet de diagnostiquer la présence d'une lésion
du sphincter anal interne et/ou externe ainsi que son étendue. Cet
examen est l'examen de référence puisque sa sensibilité et sa spécificité
sont excellentes dans le diagnostic des lésions anatomiques
sphinctériennes. Lorsqu'on suspecte une atteinte de la commande
neurologique sphinctérienne, l'examen le plus approprié pour confirmer
cette hypothèse est constitué par les tests électrophysiologiques
périnéaux, qui permettront d'orienter vers une atteinte neurologique
centrale ou périphérique (atteinte radiculaire, tronculaire, ou distale).
Il est important de rechercher une atteinte neurologique car sa présence
est un facteur de mauvais pronostic dans certains cas de cure
chirurgicale de l'incontinence anale (réparation sphinctérienne).
3/ Quelle est la gravité de l'incontinence anale ? Il existe 3 facteurs
de gravité qui entravent le pronostic du traitement de l'IA: 1/
l'hypotonie anale (inférieure à 30 cmH2O) diagnostiquée par la
manométrie anorectale; 2/ la présence d'une lésion anatomique du
sphincter anal interne diagnostiquée échographiquement; 3/ la présence
d'une pathologie neurologique diagnostiquée par les tests
électrophysiologiques périnéaux.
Le bilan minimal d'une IA comprend donc une manométrie anorectale,
une échographie endo-anale et des tests électrophysiologiques
périnéaux.
Anorectal functional testing in anal incontinence
Before any specific exploration of anal incontinence (IA), a morphological
examination including either a complete colonoscopy, or a
rectosigmoidoscopy associated with a barium rectal injection, must
be discussed among all patients presenting incontinence with an
empty rectum and that according to the clinical context (family antecedents
of colonic cancer, age, clinical symptoms….).
Specific explorations of the IA will aim to answer several questions:
1/ Does there exist a sphincter deficiency responsible for the IA?
Anorectal manometry, by determining the value of the resting anal
pressure as well as the amplitude and the duration of the voluntary
contraction of the external anal sphincter, makes it possible to know
if there is a deficiency of the anal sphincter. Clinical examination
can sometimes be as powerful as anorectal manometry in answering
this question. Nevertheless, the manometric examination also makes
it possible to evaluate the rectal functional capacity, its compliance,
and to know if a rectal dysfunction cannot be in question in the
physiopathology of the IA.
2/ What is the origin of the sphincter deficiency?
There are two possible causes to explain sphincter deficiency: an
anatomical lesion and/or a neurological disorder. Anal ultrasound
makes it possible to diagnose the presence of a lesion of the internal
anal sphincter and/or external anal sphincter as well as its extent.
This examination is the examination of reference since it has an
excellent sensitivity and specificity for the diagnosis of anatomical
lesions of the sphincters. When one suspects a neurological lesion,
the examinations most adapted to confirm this assumption are the
perineal electrophysiological tests. These tests will help to distinguish
central or peripheral neurological disorder (radiculopathy,
plexique lesions or peripheral nerve lesions). It is important to seek a
neurological disorder because its presence is a factor of bad prognosis
for certain cases of surgical treatment of anal incontinence
(sphincter repair).
3/ What is the severity of anal incontinence?
There are 3 factors of severity which decrease the prognosis of the
treatment of the IA: 1/ anal hypotonia; 2/ the presence of an anatomical
lesion of the internal anal sphincter; 3/ a neurological lesion
diagnosed by electrophysiological tests.
The minimal assessment of an IA includes anorectal manometry, an
anal ultrasound and perineal electrophysiological tests.
