Objectif. Le but de cette étude prospective était d’évaluer les résultats du traitement laparoscopique de la lithiase de la voie biliaire principale (LVBP). Patients et Méthodes. Cette étude a concerné tous les patients opérés de LVBP par laparoscopie d’octobre 1990 à décembre 2004, soit 476 cas. L’âge moyen des patients était de 63 ans (19-93 ans). Un abord transcystique (TC) fut réalisé dans 238 cas et une cholédocotomie dans 288 cas, 237 fois de première intention et 51 fois après échec d’abord transcystique. La cholédocotomie fut fermée sans drainage dans 48 % des cas. Résultats. Le taux de succès de l’abord transcystique fut de 73,5% (176/238cas). Les échecs furent traités par cholédocotomie laparoscopique dans 51 cas, et par sphinctérotomie endoscopique (SE) dans 11 cas. Le taux de succès de la cholédocotomie laparoscopique fut de 96,5%. Les 10 échecs furent traités par conversion en laparotomie dans 7 cas et par SE dans 3 cas. Le taux global de succès du traitement laparoscopique fut de 95,6 %. Des complications locales furent observées chez 4,6 % des patients et des complications générales chez 3,3 %. Le taux de mortalité s’est élevé à 1 %. Une lithiase résiduelle fut observée dans 12 cas (2,5 %). Elle fut traitée par une seconde laparoscopie avec succès dans 2 cas, et par SE dans 10 cas avec 3 échecs traités par laparotomie dans 1 cas et par laparoscopie dans 2 cas. Conclusion. Le traitement laparoscopique de la LVBP est efficace dans plus de 95, % des cas avec un faible taux de complications, en particulier chez les patients ASA 1 et 2. Il a l’avantage de traiter la lithiase en 1 seul temps opératoire.
Laparoscopic management of common bile duct stones.
Background. The aim of this prospective study was the evaluation of the results of laparoscopic management of common bile duct stones (CBDS). Material and methods. From October 1990 to December 2004, all the 476 patients who underwent laparoscopic management of CBDS were included in a prospective study. Mean age of the patients was 63 years (range: 19 to 93). Three hundred and eighty nine patients were ASA1-ASA2 and 85 patients were ASA3-ASA4. CBDS were suspected or diagnosed pre-operatively in 201 patients (63%) and at intraoperative cholangiography in 175 patients (37%). A transcystic duct extraction (TCDE) was attempted in 238 patients and a choledochotomy in 288, primarily in 237 patients and after failure of TCDE in 51 patients. A biliary drainage after choledochotomy was used in 152cases (52%). Results. TCDE was successful in 176 cases (73.5%). The 62 failures were managed by laparoscopic choledochotomy in 51 cases and by endoscopic sphincterotomy (ES) in 11 cases. Overall a choledochotomy was performed in 288 cases and was successful in 278 cases (96.5%). The 10 failures were managed by 7 conversions (2.4%) into laparotomy and 3 postoperative ES. The overall success rate was 95.6%.The morbidity rate was 7.9% with 4.6% of local complications and 3.3% of general complications. The mortality rate was 1 %. There were 12 residual stones (2.5%) which were managed by a second laparoscopy in 2 cases and ES in 10 cases with 4 failures managed by laparotomy in 1 case and laparoscopy in 2 cases. Conclusion. Laparoscopic management of CBDS is effective in more than 95 % of cases and particularly safe in ASA 1 and ASA 2 patients. It has the advantage over ES followed by cholecystectomy to be a single-stage procedure.

Le but de cette étude est d’évaluer la place de la laparoscopie dans le traitement des complications chirurgicales post-opératoires précoces de 1567 patients opérés par laparoscopie pour une pathologie digestive courante (pathologie pariétale, biliaire, appendiculaire, oeso-gastrique ou colo-rectale). Vingt sept (1.72%) des 1567 patients (20 hommes et 7 femmes, d’âge moyen : 52.26 ans) on été réopérés par laparoscopie en trois ans au 5.63 j en moyenne après la première intervention et dans 90 % des cas dans les 24 heures suivant l’apparition des troubles chez les patients rentrés à leur domicile. Il n’y a eu aucun bilan pré-opératoire dans 10 cas et réduit dans les 17 autres cas. Le taux de conversion a été de 7.4. Les suites opératoires ont été simples dans 70.4 % des cas, la morbidité a été de 29.6 % et la mortalité de 3.7 % (le seul patient muté en réanimation pour insuffisance cardiaque sévère). Il n’y a eu aucune complication spécifique liée à la laparoscopie, ni complication pariétale précoce ou tardive (abcès, éviscération ou éventration). Proposée précocement dès l’apparition de suites « non simples » ou anormalement douloureuses, la laparoscopie permet de réduire la gravité des complications chirurgicales en simplifiant la démarche diagnostique et en permettant un traitement efficace et rapide. Pour ces raisons et les avantages liés à son caractère minimal invasif, elle doit occuper une place de choix dans le traitement des complications chirurgicales post-opératoires observées après une première intervention réalisée par laparoscopie.
Laparoscopic management of early postoperative complications
following laparoscopic procedure for common digestive disease.
The aim of this study is to evaluate the place of lapararoscopy in the management of early postoperative surgical complications after a first laparoscopy for common digestive diseases. A laparoscopic procedure was performed in 27 (1.72 %) patients (20 males, 7 females, mean age: 52.26 years) of 1567 patients operated by laparoscopy during the 3 preceding years. The mean delay was 5.63 days after the first intervention and inferior to 24 hours for 90 % of the patients who had returned home. There was no complementary paraclinical examination for 10 patients and reduced examination for the 17 other patients. The rate of conversion was 7.4 %, morbidity 29.6 % and mortality 3.7 %. There was no specific complication of laparoscopy and no early or late parietal complication (abcess, evisceration or incisional hernia). Laparoscopic treatment must be proposed early when the postoperative follow-up is not simple after a first operation by laparoscopy. It allows a reduction of preoperative paraclinical examinations and an early and efficient treatment of postoperative surgical complications.

La loi du 4 mars 2002 précise qu’hormis les cas où la responsabilité d’un chirurgien ou d’un établissement est engagée, un accident médical, une affection iatrogène ou une infection nosocomiale ouvrent droit à la réparation des préjudices du patient au titre de la solidarité nationale, qu’ils soient directement imputables à des actes de prévention, de diagnostic ou de soins, lorsqu’ils ont eu pour le patient des conséquences anormales au regard de son état de santé comme de l’évolution prévisible de celle-ci, et présentent un caractère de gravité. Il convient tout d’abord d’essayer de définir ce qu’est l’accident médical. Si l’on se reporte à la loi du 4 mars 2002, et après lecture attentive de la totalité des articles, nous ne pouvons que constater qu’à aucun moment n'apparaît la définition de l’accident médical. Cette absence de définition est-elle un oubli ou est-elle volontaire ? Un droit nouveau a été créé par cette loi : le droit à l’indemnisation par la solidarité nationale des victimes d’accidents médicaux non fautifs, et donc des complications opératoires ou post-opératoires. Complications connues de tout chirurgien qui figurent depuis longtemps dans un chapitre particulier de toutes nos questions de l’ancien internat : complications de l’appendicectomie, lâchage du moignon, complications vasculaires ou urinaires d’une chirurgie de la lithiase ou complications de la chirurgie du cancer de prostate : incontinence, dysérection … L’absence de référence a permis aux conseils des victimes avec, il faut le dire, la complicité du juge qui veut indemniser tout ce qui peut l’être, tant au niveau des compagnies d’assurances que de la solidarité nationale. Ainsi les complications postopératoires ont disparu puisque tout ou presque est retenu par le juge et l’avocat comme un accident ou un aléa médical. Ce sera donc aux experts, et à eux seuls, dans leur rapport, de bien préciser ce que doit être un accident médical - imprévisible - non lié à la pathologie traitée - et indépendant de celle-ci, survenu en dehors de toute faute médicale, ce qui en définitive s’appelle aléa thérapeutique et ne pas rembourser les complications médicales, elles-mêmes aléatoires par essence, et qui devraient alors être prises en charge par une assurance volontaire prise par le patient à la veille de son intervention chirurgicale, comme cela peut se voir dans les aéroports lors des voyages aériens pour les passagers anxieux ou voulant être prévoyants. Votre Académie, quant à elle, devra se pencher sur une définition médicale de l’accident médical donnant droit à indemnisation, de façon à lever cette ambiguïté, que seuls les juges, les avocats et les experts peuvent aujourd’hui interpréter à leur guise.
Surgical responsibility and compensation of victims of medical
accidents by applying the law of March 4th 2002
The law of the 4th March 2002 stresses the fact, except in cases where the responsibility of the surgeon or the hospital is directly engaged, that a medical accident or an iatrogenous disability or a nosocomial infection give way to compensation of the encountered damage by the patient in the name of national solidarity, whether they are directly linked to prevention, diagnosis or care, as long as they resulted in abnormal consequences on the patient’s health, its foreseeable evolution and its character of severity. It is necessary to first define the aforementioned medical accident. Taking the law of the 4th of March 2002 as a reference point and after careful reading of all its articles, we can only point out that a clear definition of the medical accident is never given. Is the absence of a definition an omission or a voluntary action? A new right was created with this law: the right to compensation in the name of national solidarity of non-faulty medical accidents and as such from surgical and postoperative complications. These complications are well known by all surgeons, because they form a distinct part in every question in their final medical diploma: complications of appendicectomy, suture disunion, vascular or urinary complications of lithiasic surgery or complications of prostate cancer surgery such as incontinence or erectile dysfunction… We have to agree that this absence of a clear reference has allowed the victim’s counsellors and the judges to allow as much compensation as possible, both on the level of the insurance companies and in the name of national solidarity. Hence, postoperative complications have vanished because more or less everything is regarded by the judge and the lawyer as an accident or medical hazard. It belongs to the experts and only them to expose in their final renderings what is to be considered as a medical accident: unforeseeable, independent from the treated disease and without any medical fault. These are therefore called medical therapeutic hazards. Medical complications, themselves in essence hazardous, should be covered by a private voluntary insurance subscribed by the patient on the eve of his surgical procedure. This is already the case in airports for anxious passengers or in the case of foreseeable complications. Your academy, as such, should elaborate a clear definition of the medical accident, giving a guide to the right for compensation, in order to eliminate this ambiguity which nowadays can only be apprehended by the judges, the lawyers and the experts themselves.

Lors du soutien aux Forces Armées sur les théâtres d’opérations extérieures, les chirurgiens du Service de Santé des Armées doivent être familiarisés avec les principes de la chirurgie de guerre: mise en oeuvre rapide, matériel polyvalent, techniques simples et reproductibles. Ces principes d'exercice impliquent une formation de chirurgien généraliste en traumatologie, mais aussi en urgence digestive, thoracique et vasculaire. A la lumière de l' expérience de quatre chirurgiens de l' Hôpital d'Instruction des Armées Desgenettes de LYON, l'auteur insiste sur l'évidente nécessité de cette formation initiale pluridisciplinaire des Chirurgiens des Armées, et pointe les qualités requises dans cette pratique: capacité d'adaptation des techniques chirurgicales en environnement rustique, maîtrise de matériels de diverses origines, personnel médical multinational, autant de difficultés tout au long de la prise en charge des patients, et que tout chirurgien doit se préparer à rencontrer en mission extérieure.
Military surgeons in operations: needs of a basic training and
practice as generalist surgeon
As French Military Forces expected to be deployed in operations abroad, surgeons belonging to the Health Department of the French Armed Forces have to complete war-time surgery principles: rapid preparation and use of multi purpose material, simple surgery techniques in a surgical environment with limited capabilities; "easy to learn" surgery techniques in order to facilitate further implementation, either by local or international surgeons. Those principles of surgery practices imply a wide training in general surgery, in traumatology, but also in visceral, thoracic and vascular emergency. From the experience of four surgeons of Army Instruction Hospital DESGENETTES of LYON, the author underlines the obvious necessity of an overall initial training, for Armed Forces Military Surgeons, and points out the required qualities for such a practice: adaptability to surgical techniques with limited capacities, knowledge and ability to use a large panel of various medical items and equipments, ability to work within a multinational staff. Therefore, each Military Surgeon has to be especially prepared to military operations and to face such difficulties while carrying out his primary mission: to take care of patients.

La cryoablation et la radiofréquence sont des méthodes miniinvasives permettant, par l’application de moyens physiques au centre de la lésion, l’ablation de la tumeur. Il ne s’agit pas d’une exérèse au sens chirurgical. La voie d’abord est le plus souvent percutanée mais peut être laparoscopique dans certaines indications. Les indications sont actuellement limitées aux tumeurs rénales < 40 mm, exophytiques, à distance des éléments du hile et des structures digestives, chez des patients de plus de 70 ans ou dans le cadre de carcinome rénal héréditaire déjà opéré (maladie de von Hipppel Lindau). Les résultats des premières séries sont encourageants quoique contradictoires pour la radiofréquence du fait de la disparité en termes de matériel (sondes, générateurs, fréquence,…). Le taux de complications est faible. Un PHRC national multicentrique randomisé est désormais activé. Il doit comparer les résultats fonctionnels et carcinologiques de la radiofréquence et de la chirurgie conservatrice.
The role of ablative technologies in the treatment of renal cell
carcinoma
Ablative treatments (cryoablation or radio frequency ablation) for renal cell carcinoma aim to decrease morbidity by treating renal tumors in situ, eliminating the need for extirpation. Depending on tumor accessibility as determined by an interventional radiologist, ablative treatments were performed under percutaneous or laparoscopic guidance. The indications are virtually the same as those for nephron sparing surgery. The lesions must be less than 4 cm in diameter, peripherally located and fulfilling the CT criteria for suspected renal malignancy without evidence of metastatic disease. The clinical experience shows how renal ablation is a safe, feasible and reproductible option for minimally invasive nephron sparing surgery, offering well monitored renal tumor destruction. Although the intermediate term follow-up and literature results are promising, these techniques must still be considered to be under evaluation. Long-term evaluation is mandatory to confirm the proper indications for this surgical procedure.

La collaboration étroite de plusieurs disciplines (physique des rayonnements, biomécanique, radiologie et orthopédie de l’enfant) a permis la mise au point d’un nouvel appareil d’imagerie dénommé EOS dont les principales caractéristiques sont : - La réduction considérable des doses de rayons X (de 8 à 10 fois moins pour la radiologie bidimensionnelle, de 800 à 1000 fois moins pour la tomodensitométrie tridimensionnelle) grâce au détecteur gazeux inventé par Georges CHARPAK, qui lui a valu le prix Nobel. - L’étude du patient en position debout obtenant des clichés simultanés de face et de profil du sommet de la tête jusqu’à la plante des pieds. - La possibilité de reconstruction 3D de tous les niveaux ostéoarticulaires vérifiée aussi précise que celle obtenue par tomodensitométrie conventionnelle. L’examen est effectué en position fonctionnelle debout ou assise, ce qui n’était pas possible avec les appareils de tomodensitométrie actuellement tous réalisés en position couchée. La reconstruction 3D peut être obtenue dans des délais acceptables de 15 à 30 minutes pour un rachis complet. EOS permettra des études de la pathologie ostéo-articulaire jusquelà jamais réalisées (en particulier du rachis et des membres inférieurs) avec un examen d’ensemble de l’individu au lieu des segments fragmentés donnés jusqu’à présent par les moyens actuels, radiographies conventionnelles ou tomodensitométrie.

Physiology and Pathology with low radiation dose and standing
position
Very precise combined work between multidisciplinary partners (radiation engineers in physics, engineers in biomecanics, medical radiologists and orthopedic pediatric surgeons) lead to the concept and development of a new low dose radiation device named EOS. This device allows 3 main advantages : - Thanks to the gaseous detectors for X Rays designed by Georges CHARPAK (Nobel Price 1992) the reduction of doses necessary to obtain a good image of the skeletal system is important : 8 to 10 times less for 2D imaging, and 800 to 1000 times less for 3D reconstruction from CT scan cuts. - The accuracy of 3D reconstruction obtained is as good as a 3D reconstruction from CT scan cuts. - In addition, the patient gets its imaging in standing functional position thanks to the X-rays obtained from head to feet simultaneously AP and lateral. This is a big advantage compared to CT scan used only in lying position. - From this simultaneous AP and lateral X-rays of the whole body thanks to the 3D bone external envelop technique, the engineers in biomechanics allowed to obtain 3D reconstruction at every level of osteo-articular system and especially for spine in standing position within an acceptable period of time (15 to 30 minutes). EOS (in spite of the evolution of standing MRI) allows more precise bone reconstruction in orthopedics especially at the level of spine, pelvis and lower limbs, giving new consideration for physiology, physio-pathology and therapeutics.

But de l’étude. Tout d’abord il faut signaler que le mot " artérialisation" employé dans le contexte d’une revascularisation sous-entend que celle-ci se fait à contre-courant par l’intermédiaire des veines. Le but de cette étude était de démontrer que l’artérialisation du pied diabétique est une méthode efficace et durable, même si le pontage n’a fonctionné que temporairement. Patients et méthodes. De janvier 2001 à août 2005 nous avons revascularisé 40 membres (un cas bilatéral) chez 39 diabétiques. L’un d’eux avait déjà été artérialisé d’un côté 5 ans auparavant. Il y avait 29 hommes et 10 femmes, d’âge moyen 71 ans, (extrêmes : 53-87 ans), 36 étaient au stade IV et 3 au stade III fort du consensus européen pour l’ischémie critique, 30 avaient un diabète de type 2 ou non insulinodépendant et 9 de type 1 (OMS). L’artérialisation du pied a été réalisée par l’entremise d’un pontage veineux généralement inversé, entre une artère fonctionnelle en amont (fémorale ou poplitée) et une veine du pied en aval, de préférence la veine marginale interne avec anastomose distale en termino-latérale et modification de l’implantation du greffon au moyen d’une phlébotomie longue d’environ 40mm en moyenne (extrêmes 30-50mm) et destruction des valvules des axes veineux dorsaux à l’avant-pied. Résultats.- Nous n’avons retenu pour les résultats que les échecs et les succès. Des 39 diabétiques artérialisés, 1 est décédé à J-18 d’infarctus du myocarde avec pontage perméable. Il n’a pas été comptabilisé. Des 39 membres artérialisés chez 38 patients, 8 ont été des échecs par thrombose précoce et ont été amputés malgré de tentatives de désobstruction, 6 de la cuisse et 2 de la jambe. Les 31 restants ont été des succès (79%) dont 7 à court terme (entre 1 mois et 1 an) et 24 à moyen terme (entre 1 et 5 ans). Dans le premier groupe, 2 patients sont décédés d’infarctus du myocarde avec leur pontage occlus, 2 autres n’ont gardé leur pontage perméable que 2 et 3 mois respectivement, et les 3 derniers sont en vie au moment de cette étude, avec pontage perméable. Des 24 patients du deuxième groupe 1 est décédé d’insuffisance rénale 12 mois après son opération, ayant déjà thrombosé le pontage. Les 23 restants étaient en vie au moment de cette étude : 21 d’entre eux ont thrombosé le pontage après 10 mois en moyenne (au total 22 thromboses = 91%) en conservant les bénéfices de l’intervention. Les 2 restants ont un pontage perméable. Ils ont été suivis 25 mois en moyenne. Complications : en plus des 22 thromboses déjà mentionnées plus haut, nous avons eu 11 thromboses précoces (dans les 30 premiers jours), 8 du greffon qui ont été causes des échecs malgré des réinterventions et 3 thromboses partielles de l’anastomose distale qui, pour ne pas empêcher le fonctionnement du pontage, ont été laissées pour observation, 8 nécroses cutanées de la plaie opératoire au pied, trois progressions des nécroses au pied et un syndrome nécrotique douloureux. Nous n’avons observé aucune surcharge cardiaque, ni de varices, jusqu’au moment de l’étude. Conclusion - L’artérialisation du pied ischémique chez le diabétique est une intervention efficace et durable à moyen terme, même si le pontage n’a fonctionné que temporairement.
Temporary counter-current arterialization of the diabetic foot.
Aims of this study. First it has to be underlined that the word “arterialization” used in the context of a revascularization implies that it is fulfilled in a counter-current way, by means of veins. The aim of this study is to prove that the arterialization of the diabetic foot is an efficient and durable method, even if the bypass has only been temporarily functional. Patients and Methods. From January 2001 to August 2005 we have revascularized 39 diabetic patients, one of them bilateral. There were 29 men and 10 women. Their mean age was 71 years (range: 53-87). There were 36 stage IV patients and 3 stage III patients. Thirty non insulinodependant diabetes type 2, and 9 type 1 (OMS). The arterialization of the foot has been performed using a venous bypass generally placed reversed between a functional artery upstream (femoral or popliteal) and a vein of the foot downstream, giving the preference to the medial marginal vein, anastomosing it distally in a termino-lateral way, modifiying the implantation of the graft by means of a long phlebotomy of about 40 mm (extremes 30- 50 mm) and destruction of the valves of the venous axis of the foot. Results. We have only retained the failures and the successes. From among 39 arterialized diabetics, one died on day 18 of myocardial infarction with patent by-pass; he has not been included. Among the 39 limbs in the 38 remaining patients, 8 have been immediate failures and have been amputated, 6 of them at the thigh and 2 at the leg. The others 31 patients have been successes (79%): 7 of them at short term (between 1 and 12 months) and 24 at medium term (between 1 and 5 years).In the short term group, 2 died of myocardial infarction with their bypass occluded. Two others had their bypass patent only 2 and 3 months respectively and the 3 remaining are still alive with a patent bypass. The 24 patients of the medium term group have been followed during an average of 25 months. One patient died of renal failure 12 months after surgery with his bypass occluded. The 23 remaining patients are still alive. There were 21 thrombosis of the graft in approximately 10 months (total: 22 thrombosis = 91%) with preservation of the benefits of the operation. The last two patients have their bypass patent. The most frequent complication, during the first 30 days, was early thrombosis, especially during the first week after operation. We had 11 early thromboses, 8 of them were failures in spite of repeated operations and 3 partial thrombosis of the distal anastomosis which were left in observation in order not to hinder the functioning of the bypass. There were 8 cutaneous necroses and 1 necrotic pain syndrome. There was no cardiac overloading, nor varicose veins up to this date. Conclusion. Arterialization of the ischemic foot in diabetic patients is an efficient and durable operation in medium term successes, even if the bypass has only been temporarily functional.

La mobilité de nos structures nous est tellement intrinsèque, naturelle, qu’elle n’appelle pas d’interrogation. Le simple geste de pouvoir soulever la peau, l’observer se redraper, reprendre sa forme et sa texture initiale en quelques secondes est certes très banal mais en fait, très interrogatif quand on pense à tous les éléments entrant en jeu. Le constat est le même lorsque l’on ferme les doigts et que l’on pense à la progression du tendon fléchisseur tout au long de la paume sans traduction externe. Pendant des décennies, les explications scientifiques se sont bornées à la notion du concept d’élasticité ou de l’existence de tissu conjonctif lâche (connective tissue) lamellisé avec plus ou moins un espace virtuel, explications dont la biomécanique est plus que floue. En dehors de ces concepts anciens, depuis plus de 50 ans, la recherche scientifique est passée au niveau microscopique et a abandonné le concept global, mésosphérique. La dissection chirurgicale in vivo permet de constater qu’il n’ y a que des connections tissulaires, une véritable continuité histologique sans séparations nettes que ce soit entre la peau et l’hypoderme, les vaisseaux, puis l’aponévrose et le muscle. On discerne partout des structures qui assurent le glissement, que ce soit entre l’aponévrose musculaire, les structures graisseuses, puis le derme. Les auteurs, à l’occasion de leurs études sur les systèmes de glissement entre organes, et en particulier au niveau tendineux ont constaté un type de structures, constituées de filins, haubans, câbles, voilages, qu’ ils ont appelé le concept de Système Collagénique Multimicrovacuolaire d’Absorption ynamique. M.C.D.A.S. Ce système est d’organisation totalement chaotique et de fonctionnement très éloigné des analyses mécaniques traditionnelles. L’unité fonctionnelle du glissement des structures déterminée par le croisement dans les 3 dimensions de l’espace est la microvacuole, de forme polyédrique et dont l’armature collagénique est de type I ou IIII et le contenu constitué de protéoglycoaminoglycanes. L’acceptation du concept vacuolaire permet aussi de mieux définir des états de la matière au décours d’une vie comme l’oedème, l’obésité, le vieillissement et enfin l’inflammation. Ce système de glissement se retrouve dans tout le corps et semble être la trame tissulaire organisatrice globalisante. Il impose une vision plus holistique.
The multi microvacuolar collagenic dynamic absorbing system : a new concept
The mobility of our body structures is so intrinsic and natural to us that we tend to take it for granted. The very fact of being able to pinch your skin and lift it, then let it go and see it return to its initial shape and texture in just a few seconds may seem banal enough until you begin to think of all the elements involved. The same is true when you bend your fingers and think of the movement of the flexor tendon across the palm without external translation. For decades, scientists thought that the skin was simply an elastic structure with loose connective tissue and a more or less virtual space. However, in biomechanical terms, this explanation is very vague. These old concepts, developed more than 50 years ago, have evolved thanks to the impact of research at the microscopic level, and the global, mesospheric concept has been abandoned. And yet, surgical dissection in vivo demonstrates that there are only tissue connections, simply a histological continuum without any clear separation between skin and hypodermis, the vessels, the aponeurosis and the muscles. In fact, visible everywhere are structures which ensure a gliding movement between the aponeurosis, the fat structures and the dermis. As they studied this system of gliding between the various organs, in particular at the level of the tendons, the authors noted the existence of a type of system composed of cables and veil-like structures that they term the Multimicrovacuolar Collagen Dynamic Absorption System (MVCAS). This system looks totally chaotic in organization and seems to function in a manner far removed from traditional mechanical structures. The functional unity of this sliding system is dependent upon a polyhedral three-dimensional crisscrossing in space of the microvacuoles, whose collagen envelope is type 1 or type 4 and whose content is made up of proteoglycoaminoglycans. The dynamic of this multimicrovacuolar system allows all of the subtle movements that occur within the body, thanks to its pre-stressed nature and the molecular fusion-scission-dilacerations that it is capable of. In this way, the system is mobile, can move quickly and interdependently, and is able to adapt is plasticity. This notion of microvacuoles is a fascinating one because it provides an explanation for the system’s space-filling ability. The matter is composed of elements. However, although they seem to be arranged in a haphazard manner, this is not the case. In fact, they occupy space in an optimal manner. If we accept this notion of microvacuoles, then it becomes possible to explain certain pathologies occurring with age, such as edema, obesity, aging and inflammation. This sliding system is to be found everywhere in the body and would seem to be the basic network of tissue organization. For this reason, it should be thought of in global terms. Since it constitutes the inseparable link and occurs in all living structures and at many levels, could it be that it the basic architectural design of Life ?

La voie coelioscopique permet : la réalisation de tous les gestes que l’on réalisait par laparotomie, le traitement de l’étage postérieur impossible par laparotomie. Notre technique comporte : une hystérectomie sub totale, la mise en place d’une prothèse fixée en arrière aux releveur de l’anus et dans l’espace vésico vaginal, la fixation de cette prothèse sur le promontoire, la dissection de l’espace de Retzius avec réalisation d’un Burch et d’une réparation para vaginale par suture ou par prothèse non résorbable. Les résultats de cette méthode, à partir d’une série continue de 138 patientes opérées entre 1998 et 2003, montrent des résultats anatomiques satisfaisants dans 90% des cas avec un recul moyen de 30 mois. Le pourcentage de complications per et post opératoires est de 5,8% et 2,9%. Neuf patientes ont présenté une complication due à la prothèse. Dans 7 cas il s’agissait d’une exposition de prothèse traitée par un geste simple de parage suture par voie vaginale. Deux prothèses ont été retirées, une en raison d’une fistule vésico vaginale, l’autre d’une spondylodiscite. Ces résultats prometteurs doivent être confirmés à long terme et par d’autres équipes.
Laparoscopic management of prolapse using a mesh in the recto vaginal and in the vesico vaginal spaces.
In the management of genital prolapse, the laparoscopic approach allows all the procedures previously achieved by laparotomy, and the treatment of the posterior part of the defect which was previously impossible by an abdominal route. Our technique involves a supracervical hysterectomy, the insertion of a mesh posteriorly sutured to the levator ani and to the vagina and anteriorly in the vesico vaginal space and to the uterine cervix. The mesh is then fixed to the promontorium. In the Retzius space a Burch procedure and a paravaginal repair are performed. The preliminary results of this approach are presented in a series of 138 patients with a mean follow-up of 30 months. The anatomical results are satisfactory in 90% of the cases. The intra and postoperative complication rates were respectively 5%, 8% and 2,9%. Nine complications related to the mesh occurred. In 7 cases a vaginal erosion was treated by vaginal excision and suture. Two meshes had to be removed, one because of vesico vaginal fistula, successfully treated by laparoscopy, and one for spondylodiscitis. These promising results will have to be confirmed with a longer follow-up and by results obtained in other groups using similar endoscopic approaches.
