L’oesophagectomie reste le traitement de référence des cancers de
l’oesophage opérables. Après résection complète (R0), la survie est
significativement allongée par rapport aux résections incomplètes
avec résidus microscopiques (R1) ou macroscopiques (R2). Le but
de ce travail était d’identifier de façon prospective des facteurs
prédictifs de résection complète des cancers de l’oesophage
opérables.
De janvier 1995 à janvier 2002, 372 patients ont bénéficié d’une
oesophagectomie curative pour cancer. Une résection R0 a été
réalisée chez 304 patients (81,7%), R1 chez 28 patients (7,5%) et
R2 chez 40 patients (10,8%). Seize variables pré- et peropératoires
ont été incluses dans des analyses uni et multivariée.
Les facteurs prédictifs de résection R0 étaient : l’absence de
désaxation oesophagienne sur le transit oesogastroduodénal
(P=0,019) et une réponse partielle ou complète morphologique à la
radiochimiothérapie néoadjuvante (P=0,042). Trois groupes de
patients ont alors été identifiés : Groupe 1 : pas de désaxation
oesophagienne au transit (n=253) ; Groupe 2 : désaxation
oesophagienne au transit et réponse partielle ou complète à la
radiochimiothérapie néoadjuvante (n=66) ; Groupe 3 : désaxation
oesophagienne au transit et absence de réponse ou pas de traitement
néoadjuvant (n=53). Les taux de résection R0 et de survie à 5 ans
étaient respectivement de 90,1%, 74,2% et 50,9% et de 46%, 37%
et 0% (P<0,001).
La résection complète du cancer de l’oesophage est prévisible. La
désaxation oesophagienne au transit et la réponse morphologique au
traitement néoadjuvant sont des variables disponibles pour chaque
patient lors de la consultation de chirurgie.
Operable oesophageal cancer: preoperative prediction of
complete resection
Esophagectomy remains a standard treatment for patients with
resectable esophageal cancer. After complete resection (R0),
survival is significantly longer than after incomplete resection, with
microscopic (R1) or macroscopic (R2) penetration. The purpose of
this work was to identify prospectively the factors predictive of
complete resection of operable esophageal cancers.
Between January 1995 and January 2002, 372 patients with

and R2 in 40 (10.8%). Univariate and multivariate analysis included
16 preoperative and operative parameters.
Factors predictive of complete resection R0 were : absence of any
modification of the esophageal axis on the barium swallow
(P=0.019) and a partial or complete response to preoperative
radiochemotherapy (P=0.042). Three groups of patients were
identified. Group 1 : no deviation of the axis on the barium swallow
(n=253) ; Group 2 : deviation of the axis on the barium swallow and
partial or complete response to radiochemotherapy (n=66) ; Group
3 : deviation of the axis on the barium swallow and no response to
radiochemotherapy or no preoperative treatment (n=53). Rates of
R0 resection were 90.1%, 74.2% and 50.9% and 5-year actuarial
survival 46%, 37% and 0% respectively (P<0.001).
Complete resection of esophageal cancer is predictable. Deviation
axis on the barium swallow and morphological response to
neoadjuvant radiochemotherapy are variables available for all
patients at onset of therapeutic management.
